Authorization Form to Change Address, Beneficiary, etc.
Securing a Loan at BASEFCU
Print the form, and fill it out. We will add your member number.
Include the following:
1. Completed application form
2. Two pay stubs or other proofs of income (bank statement showing direct deposit etc.) showing you have been employed 90 days.
3. If you are including child support in your proof of income, you must show proof of the age of the child to show how long the income will be available and proof of the court ordered amount.

You must have been employed for 90 days and show 2 pay stubs or other proofs of income (bank statement showing direct deposit etc.)
If the address on your photo ID does not match what we have, you must show proof of residence (such as utility bill, current pay stub)
In order to include child support in proof of income you must show proof of the age of the child to show how long the income will be available, and proof of the court ordered amount.
